Appointments

New Patient

Our practice is accepting new patients who are currently working with a therapist.  Appointment requests are reviewed by our providers and are considered on a case by case basis.  Please be aware that there are limitations on who can be seen in our office. Requests for new patient consultations may take a week or more to process during our busy periods. We ask that you please be patient and do not submit multiple requests as this will only delay our efforts to make appropriate contact with all of those waiting.

To schedule a follow up appointment, you may log into your patient portal to view appointment availability or call during phone hours to discuss appointment options.

Request an Initial Evaluation

Email admin@clearwateraustin.com with the person’s full name, date of birth & email address who is wanting the appointment.  An “Appointment Request Form” will be emailed to them. If request is for a minor, please provide your name in the email.

 

Initial Evaluation Process

  • An “Appointment Request Form” will be emailed to you.  Be sure to check your spam folder for the form.  NOTE: Link to access the form is time sensitive and expires 5 days from the day it is sent.
  • Complete the form and include insurance information if you have active coverage.  This will allow us to obtain your out-patient mental health benefits.
  • The form will be reviewed by the provider you specified.  If that provider is not able to see you, we will pass it to another PMHNP in our practice. 
  • Upon approval to schedule, you will receive two emails: 1) New Patient Forms via IntakeQ. 2) Email to set up your patient portal and complete portal documents.  All forms and portal documents are required to be completed before any appointments will be scheduled.  
  • Please do not request an appointment on the portal, we will contact you to schedule.
  • We will call or email you if our practice is not able to accommodate your request for an appointment. 
  • If you have not heard from us within 3-4 days after submitting the form/s, please call or email us.
