1. Fill out this Contact Form or email admin@clearwateraustin.com with:
1. Full name, 2. DOB and 3. Email address – ALL 3 ARE REQUIRED. *For minors, include your name as well.
2. Check your email (and spam) for a link to set up your Patient Portal. Link sent from TherapyPortal.com.
3. Set up your Patient Portal. Complete the Appointment Request Form and include insurance information if applicable.
Next Steps
4. Providers review your Appointment Request Form. If appropriate to schedule a consult, you will receive: New Patient Forms via TherapyPortal.com.
5. Complete all portal documents.
6. We will contact you to schedule an appointment. If we cannot accommodate your request, we will notify you.

Returning Patient
To schedule a follow up appointment, you may log into your patient portal to view appointment availability or call during phone hours to discuss appointment options.

Telehealth Information
Clear Water Psychiatry and Wellness offers appointments through TeleHealth. A TeleHealth appointment provides a convenient way for patients to receive mental health treatment, whether you are mobile-challenged, lack transportation, or have travel time constraints.
Our providers rely on the ability to connect emotionally and to have active communication with each patient. This makes the addition of a secure video chat platform a desired tool here at Clear Water Psychiatry for new and established patients.
TeleHealth provides healthcare to patients who are geographically separated from providers. With any new service you will need to verify whether TeleHealth is covered under your plan. Most insurance plans cover TeleHealth visits under the same benefits as an in-office mental health visit, applying your copay, deductible, and/or coinsurance.
Insurance Information
We participate with some insurance plans, self pay is also an option. Always check with your Insurance carrier to determine plan benefits and network. We do not participate in any Medicare Advantage Plans.
